Disclosure upon Application

The disclosure of government information upon application means that a citizen, a legal person or other organizations may apply for the access to the government information outside that has made public from the Municipal Health Bureau based on their special needs of their own production, life and scientific research.
I. The Accepting Institution for Applications
Xi'an No.1 Hospital has formally accepted the applications for the disclosure of government information upon application since June 1, 2009, and the accepting institution is the Office of Xi'an No.1 Hospital of which working hours is every weekday morning from 8: 30-12: 00 and afternoon from 14: 00-18: 00 (on July and August, from 15: 00-18: 00).

II. Filing of Application
Should any citizen, legal persons or other organizations apply for government information outside that has made public from the Xi'an No.1 Hospital due to their own production, life, research and other special needs, please complete the "Information Disclosure Application Form of Xi'an No.1 Hospital" (see Annex) first to apply for the access to relevant government information. The electronic version of application forms can be downloaded in the website of Xi'an No.1 Hospital (http://www.xadyyy.com), or you can collect at the Office of Xi'an No.1 Hospital.
III. Submission of Application
　　 (A) Online Submission.
　　 　　 Applicants may fill out online the electronic version of the "Application Form" on the website of Xi'an No.1 Hospital and submit it to Xi'an No.1 Hospital for application acceptance, or download and fill out the "Application Form" before sent it to the Office of Xi'an No.1 Hospital by e-mail.  Please indicate the message header as "Information Disclosure Application to Xi'an No.1 Hospital".
　　　　 (B) By Mail.
Applicants may sent the "Application Form" to the accepting institution of Xi'an No.1 Hospital by mail (On the top left corner of the envelope, please indicate "Information Disclosure Application to Xi'an No.1 Hospital").
(C) Written Application. Citizens, legal persons or other organizations need to provide the information related to registration and results inquiries of qualification tests of themselves, etc. also need to bring their valid identity document to the Information Disclosure Office of Xi'an No.1 Hospital, and fill out the "Application Form" and go through the procedures.
IV. Application Processing
　　 Government Information Disclosure Office of Xi'an No.1 Hospital will register the "Application Form" within 15 working days after receiving it and will offer following replies (The time can be extended by 15 working days which will inform the applicant in special circumstances):
　　 　 (A) If the information applied by applicants belongs to the scope of disclosure information, applicants will be informed of the ways and means by which such applicants can get the inform in writing;
　　 　 (B) If the information applied by applicants does not belong to the scope of disclosure information, the applicants will be informed of the reasons why such information cannot be disclosed;
　　      (C) If the information applied by applicants does not belong to the disclosure scope of Xi'an No.1 Hospital or does not exist, the applicants would be informed of such facts in writing. If the government authorities that have rights to disclose the information can be determined, the authorities would be informed of the name and contact information of such authorities.
              (D) If the content of an application is not clear, the Information Disclosure Office of Xi'an No.1 Hospital would inform the applicant to make amendments and supplements to such application.
V. Charge Management
　     　 (A) If applicants request to receive the government information disclosure upon application in electronic form, the Government Information Disclosure Office of Municipal Health Bureau would offer for free;
              (B) If applicants request to receive the government information disclosure upon application by other forms, the charges would follow the Charge Standards established by the price administrative department and the financial sector of State Council and provincial and municipal authorities.
